Incomplete Consent Status

Incomplete: An Incomplete consent status could generate due to several factors:

  • Inconsistent Data/Name Changes: This is normally when the name on a consent form doesn't match the individual's legal name in the system. First, update the person’s profile. If that does not work, email qualification.review@fssa.in.gov with documentation of the current and previous name changes. If the information submitted on the last I-LEAD consent form contains incorrect data or if the I-LEAD account information is tangled with someone else’s, contact the OECOSL Child Care Support Line at 1.800.299.1627 for technical support.
  • Missing/Incorrect Fingerprints: If the correct FBI fingerprint receipt number was never submitted with the last consent form, redo the consent form in your I-LEAD account with this missing information. The “Begin Consent Form” button should reappear in your account after midnight if you submitted a consent form that same day. Once the consent form is submitted, a “Submitted” status will appear instead of “Incomplete.” Processing can take up to 14 business days as long as all paperwork was received by qualification.review@fssa.in.gov.
  • Pending Disqualification: An educator could be marked as Incomplete if results of a pending disqualification need further review by qualification.review@fssa.in.gov. If the status later changes to “Disqualified” instead of “Incomplete,” the educator will be prompted to complete a new consent form via “Begin Consent Form." Upon redoing the consent form, the educator must upload supporting documents that dispute the disqualification status and show they are still eligible to work in a child care setting.

  • Previous Out-of-State Resident: Educators who lived in another state within the last five years may receive an email from the background checks department directing them to take additional steps. The educator should look for an email from outofstatechecks@fssa.in.gov to submit paperwork giving licensing permission to run the background checks from another state. There may be more than one form attached to the email if an educator lived in multiple states within the past five years. All requests need to be completed and returned to outofstatechecks@fssa.in.gov for processing.

Typically, those with an incomplete status will receive a letter within 7-10 business days to indicate the reason. If you don’t receive the letter within this time frame, contact the OECOSL Background Checks Department at qualification.review@fssa.in.gov for further instructions.
