Provider Information

This section of the Provider Details page shows information related to the license. These fields include:

  • Provider Name: Name of the child care program (Can be fixed with an update application.)
  • License Number: Number assigned to the child care program when an application is signed and submitted.
  • Provider Type: The type of license for the child care program.
  • Address: The current address of the child care program associated with the license.
  • Provider Status: The status of the license.
  • Consultant Notes: This field only generates after an application has been submitted.

Consultant Notes
Monitor your email and the Provider Details page for messages from your consultant. If an application requires a change, a consultant note will be added to the Provider Information box next to Provider Admins. The application may also be in "Edit" mode in the Provider Application section. Consultants will normally schedule an in-person inspection of the premises once all consent forms and licensing paperwork are completed online. Renewed licenses are approved once all requirements are met, and the new expiration date shows on the I-LEAD Dashboard.

(Note: Wait approximately 24 hours for the “Edit Application” button after the licensing consultant marks the application as “Incomplete” in the system. Once the “Edit Application” button is available, you can make the changes. Then sign and submit the application for your consultant to view the results.)
